Plantar Fasciitis

What is Plantar Fasciitis ?

Plantar fasciitis (PLAN-tur fas-e-I-tis) is one of the most common causes of heel pain. It involves inflammation of a thick band of tissue that runs across the bottom of your foot and connects your heel bone to your toes (plantar fascia).

Plantar fasciitis commonly causes stabbing pain that usually occurs with your first steps in the morning. As you get up and move more, the pain normally decreases, but it might return after long periods of standing or after rising from sitting.

Plantar fasciitis is more common in runners. In addition, people who are overweight and those who wear shoes with inadequate support have an increased risk of plantar fasciitis.

Plantar Fasciitis Symptoms

  Plantar fasciitis typically causes a stabbing pain in the bottom of your foot near the heel. The pain is usually the worst with the first few steps after awakening, although it can also be triggered by long periods of standing or rising from sitting. The pain is usually worse after exercise, not during it.

Plantar Fasciitis Causes

Under normal circumstances, your plantar fascia acts like a shock-absorbing bowstring, supporting the arch in your foot. If tension and stress on that bowstring become too great, small tears can arise in the fascia. Repetitive stretching and tearing can cause the fascia to become irritated or inflamed, though in many cases of plantar fasciitis, the cause isn’t clear.

Risk factors

Though plantar fasciitis can arise without an obvious cause, factors that can increase your risk of developing plantar fasciitis include:

  • Age. Plantar fasciitis is most common between the ages of 40 and 60.
  • Certain types of exercise. Activities that place a lot of stress on your heel and attached tissue — such as long-distance running, ballistic jumping activities, ballet dancing and aerobic dance — can contribute to an earlier onset of plantar fasciitis.
  • Foot mechanics. Being flat-footed, having a high arch or even having an abnormal pattern of walking can affect the way weight is distributed when you’re standing and put added stress on the plantar fascia.
  • Obesity. Excess pounds put extra stress on your plantar fascia.
  • Occupations that keep you on your feet. Factory workers, teachers and others who spend most of their work hours walking or standing on hard surfaces can damage their plantar fascia.

Black ToeWhat is a Black Toe ?

Black toe is a relatively common condition among vigorous walkers, runners, hikers, and participants in other sports and activities where the feet are subjected to stress and strain, such as football, basketball, track, baseball and soccer. The condition generally is the result of impact trauma, particularly in the big toe.

People who walk and run on hard surfaces such as concrete and asphalt are more likely to get black toe than those who exercise on natural surfaces like grass or softer man-made surfaces. Hikers can experience black toe because of the toes banging against the front area of their shoes or boots, especially on downhill sections of trails.

Symptoms of Black Toe

The most obvious symptom of black toe is a black or dark blue color on part or all the toenail caused by bruising or pooling of blood under the nail. Given that black toe most frequently is a result of trauma to the toe or the nail, it may be accompanied by pain or discomfort that usually diminishes within a few days. Black toe can also occur after lengthy periods of vigorous activity even without noticeable trauma or pain.

In some cases, discoloration of the toenail can result from causes other than black toe, some of which are benign. Others, such as the rare disorder, subungual melanoma, are cause for concern

The most common cause of black toe is repeated trauma from the toe banging against the end of the shoe. Since the big toe is usually the longest toe, the banging produces bleeding and bruising of the nail, causing it to become discolored (often blue and then black). The banging of the toe against the front of the shoe or boot is usually a sign that the shoes are too short, or that they fit too tightly and do not allow sufficient room for the feet to move dynamically during walking and running.

Other causes may include the following:

  •     Sudden trauma or injury to the toe or toenail.
  •     Fungal infection.
  •     “Morton’s toe,” a condition in which an individual’s second toe is longer than the big toe.
  •     Rarely, a subungual (occurring under the toenail) melanoma, a form of skin cancer.

foot and ankleIn our experience, with the foot and ankle, we witness children between 10 and 14 experience heel pain as a result of inflammation of the growth plate at the back of the heel. The Achilles tendon, which is the largest and strongest tendon in the body, inserts into the back of the heel bone, just behind the growth plate. Kids in track, soccer, and gymnastics will often develop this condition, known as “Sever’s disease”.

Foot and Ankle Problems in Georgia Sports

In addition, Podiatry Group of Georgia points out that runners and those frequently walking on a treadmill can both develop a stress fracture. This occurs when someone abruptly increases their mileage or number of times per week they are exercising. A stress fracture is usually an overuse type of injury that produces acute bone swelling leading to a crack in the bone. They are treated with a period of non-weight bearing to the affected area, and usually require crutches and a cast or cast boot until the bone heals.

Tendinitis can develop with an increase in mileage or with any repetitive activity. In the foot and ankle, we see tendonitis develop in the Achilles tendon, or on the inside or outside of the ankle depending on the foot type and mechanics. If you have an area of your ankle that swells up with activity, and becomes sore, you probably have a case of tendonitis. Controlling foot mechanics with the proper shoes and arch supports can often prevent or be used to treat a case of tendonitis.

There are a lot of things you can do to prevent injuries such as stretching your foot and ankle before and after exercising. Finding the right shoes is important as well. When beginning an exercise program, make sure you start out slowly. If you have flat arches, it is wise to purchase arch supports or orthotics sized for your needs. Whenever you experience any type of pain, ice the area or seek medical help if it remains.

Stretches for the Feet and Ankles

Ankle and Foot Stretches

The next time you watch an exercise program, pay special attention to the time spent on stretching the feet and ankles.  It’s likely that your favorite exercise guru completely ignores these areas of the body.  This seems counterintuitive when you consider that your feet are bearing up to four times or more the body’s weight when you do jumping jacks.  While the research varies on the benefits of stretching, one of the primary goals of this activity is injury prevention.

Note that the calves are also an important piece to the stretching puzzle and good foot health.  These muscles are connected to the feet by the Achilles tendon, and they play a role in conditions such as plantar fasciitis and Achilles tendonitis.

Stretching not only aids in the prevention of injury, but it also helps during recovery.  Whether you’ve experienced an ankle fracture or foot surgery, home exercises or physical therapy are often a part of the after-care plan.  Restoring strength and flexibility to the affected area is the primary goal.  Focused stretching also brings the body’s natural healing process to the area through increased blood flow.

What do I need to know about stretching?

Stretches for the Feet and Ankles Many people see stretching as the beginning of their workout routine, but it should really be phase two.  First, take a quick walk, or even jog in place to get your body warmed up prior to completing your regimen of stretches.  Remember that stretching should not cause pain.  If you experience discomfort, you should pull back to a more comfortable point.

This process could take up a large period if every muscle of the body was addressed.  Instead, break the body down into areas of focus, and spend more time on the muscles that you will use during your regular workout.  To prevent tearing, keep the targeted muscle still during the stretch.  It’s always best to speak with your doctor before beginning any type of exercise plan.

What are some common stretches for the feet, ankles, and calf muscles?

Dr. Gephart prescribe stretching exercises for various conditions after an evaluation.  However, there are some general stretching exercises that can benefit anyone.

Stretch the toes–While sitting, place your feet flat on the floor and move your toes apart.  Hold and repeat.  Next, grab a towel and lay it on the floor.  Use your toes to gather it up, and then lift it.  Finally, loop the towel around your toes and gently pull toward your body.

Work out arch pain—Spend ten to fifteen seconds walking on your toes and complete eight reps.  Using a golf ball or tennis ball, give yourself a foot massage by rolling the item back and forth underneath your foot. For additional benefits, try a frozen water bottle.

Don’t avoid the ankles—Sit in your favorite chair and flex your feet slowly.  After ten reps, point the toes and hold.  Do these ten times as well.  You can also stretch the ankles by going downward and inward and holding that position.  Now, try going upward and outward.

Focus on the forgettable calves—The calves are often forgotten, but they are an important part of the overall health of your feet.  Use a step for one of the most popular stretching exercises.  Stand with your heels over the edge, and let the heels drop.  Now, stand in front of a chair back with your feet apart.  Rise up on one foot and lift the heel as far as possible. Lower and repeat ten times.

It isn’t terribly unusual to think that adults are the only ones who develop bunions. After all, there is a perception that this condition is closely linked to stilettos and pumps – shoes for grown-ups. It’s important to understand that children can develop them, too. Bunion Basics

Treatment for Painful Bunions in Children The first step in discussing pediatric bunions is to become familiar with the condition.  A bunion develops when the great toe (the hallux) drifts outwards in the direction of the little toe. A misalignment occurs at the metatarsophalangeal (MTP) joint, the joint where the toe meets the foot. The phalanges (toe bones) lean outwards and the metatarsal, the bone running lengthwise the foot, leans inward towards the other foot.

The bunion is the enlargement of the joint that protrudes along the inner edge of the foot.  In adults, there can be an increase in size of the bone. In the case of pediatric bunions, this is purely structural. The pain occurs by irritation of surrounding bone and nerve. This affects a person’s ability to walk and comfortably wear shoes.

Pediatric Bunions

In adults, the pain is often exacerbated by wearing fashionable or high-heeled shoes (obviously not the case with pediatric patients).  Yet all patients frequently have pain, indicating that shoes alone are not the culprits.  Interestingly, female children are more likely to have bunions than males.

Conservative Treatment Options

As with adult cases of bunions, the treatment objectives are twofold – address structural problems and manage symptoms. Managing the symptoms can often be achieved with the use of padding, changing activities, and avoiding shoes that are too narrow. Regarding structural treatments, toe spacers, toe splints, orthotics, and supportive shoe gear may be part of the plan. Orthotic devices can be particularly beneficial at bridging the gap between the two objectives.

Pediatric Bunion Surgery

Performing surgery for cases of pediatric bunions is generally reserved as a measure to be used only when other treatments are unsuccessful, and the child is in pain. Part of the concern with operating on children at early ages is the fact that their bones are still growing, and accidental damage to a growth plate could disrupt a bone’s ability to grow.

That said, there are instances where surgical procedures offer your child the best opportunity to find pain relief and correct the condition. Typically, we would not want to operate on a girl who is younger than 13-15 or a boy who is 15-17, since this is about when they reach skeletal maturity.

The good news about pediatric bunion surgery is that, when it is necessary, the procedure is often quite successful in realigning the bone and correcting the bone structure.

The balls of your feet—those padded areas on the bottom of your feet between the toes and the arch—are surprisingly complex under the surface. Five long metatarsal bones occupy a tight space with a pair of sesamoid bones, muscles, ligaments, tendons, nerves, and blood vessels. Considering how much stress and impact force this area of the foot must endure, pain is unsurprisingly common.

While temporary aches are fairly normal, persistent pain is a sign that something deeper is wrong and requires further examination and treatment. The bottom of a foot Symptoms of Ball of Foot Pain.

Pain in this region of the foot is also known as metatarsalgia, which is a “catch-all” term that covers the symptoms of many different types of conditions and injuries. Some of the most common symptoms include:

  • Pain that can be described as sharp, burning, aching, stabbing, or shooting
  • Symptoms worsen with activity and improve somewhat with rest
  • Tingling or numbness that extends into the toes
  • Feeling like you’re constantly standing on a small pebble or fold in your sock

The pain is usually centered near the base of the big toe—to be specific, around the head of the first long metatarsal bone. That’s because this is the part of the ball that handles the greatest amount of weight and force when you walk, run, and jump.

What Causes Ball of Foot Pain?

There are dozens of potential underlying factors and causes that could lead to metatarsalgia. When you come for your appointment, we’ll help you ascertain the most likely culprits. Some of the most common include:

  1. Structural problems with the feet, such as flat arches, bunions, hammertoes, or abnormally long metatarsal bones.
  2. Participation in high-impact sports (running and jumping), dance, or occupations that require you to be on your feet constantly
  3. Rapid increase in physical activity or change in training routine
  4. Wearing poorly fitting shoes (especially high heels)
  5. Development of stress fractures in the metatarsal bones
  6. Formation of a neuroma (swollen nerve tissue) near the base of the toes
  7. Inflammation or fracture of the sesamoids, small bones that act like pulleys for your big toe
  8. Obesity

How to Treat Ball of Foot Pain

If pain is severe, disrupting your activities, or lingers for longer than a week or so, you should make an appointment with the Dallas Podiatry Works team. An examination and discussion with your physician (using X-rays if necessary) will help reveal both treatment methods and prevention strategies that may be beneficial.

Most cases and causes of metatarsalgia can be corrected through conservative therapies, such as:

  • RICE therapy (rest, ice, compression, elevation)
  • Temporarily avoiding activities that cause spikes in pain
  • Physical therapy
  • Wearing more appropriate shoes
  • Using metatarsal pads, arch supports, taping or splinting the big toe, custom orthotics, or other inserts as necessary to relieve pain and pressure
  • Over-the-counter pain relievers
  • Injections

Occasionally, surgery may be required to correct an underlying structural problem, such as a bunion or flat feet, or to remove a neuroma. This will reduce the pressure on the balls of the feet to eliminate the pain.
