Whether you’re slogging through deep snow and sub-zero temperatures in the north, or contending with dampness, chill, and muddy conditions in the south, it’s important to take care of your feet all winter long. You’ll want them to be healthy and ready for action when spring finally arrives.

Most Americans will have walked 75,000 miles by the time they turn 50. Is it little wonder, then, that APMA’s 2010 foot health survey found that foot pain affects the daily activities—walking, exercising, or standing for long periods of time—of a majority of Americans?

“Each season presents unique challenges to foot health,” said Matthew Garoufalis, DPM, a podiatrist and APMA past-president. “Surveys and research tell us that foot health is intrinsic to overall health, so protecting feet all year long is vital to our overall well-being.”

APMA offers some advice for keeping feet healthy in common winter scenarios:

  • Winter is skiing and snowboarding season, activities enjoyed by nearly 10 million Americans, according to the National Ski Areas Association. Never ski or snowboard in footwear other than ski boots specifically designed for that purpose. Make sure your boots fit properly; you should be able to wiggle your toes, but the boots should immobilize the heel, instep, and ball of your foot. You can use orthotics (support devices that go inside shoes) to help control the foot’s movement inside ski boots or ice skates.
  • Committed runners don’t need to let the cold stop them. A variety of warm, light-weight, moisture-wicking active wear available at most running or sporting goods stores helps ensure runners stay warm and dry in bitter temperatures. However, some runners may compensate for icy conditions by altering how their foot strikes the ground. Instead of changing your footstrike pattern, shorten your stride to help maintain stability. And remember, it’s more important than ever to stretch before you begin your run. Cold weather can make you less flexible in winter than you are in summer, so it’s important to warm muscles up before running.
  • Boots are must-have footwear in winter climates, especially when dealing with winter precipitation. Between the waterproof material of the boots themselves and the warm socks you wear to keep toes toasty, you may find your feet sweat a lot. Damp, sweaty feet can chill more easily and are more prone to bacterial infections. To keep feet clean and dry, consider using foot powder inside socks and incorporating extra foot baths into your foot care regimen this winter.
  • Be size smart. It may be tempting to buy pricey specialty footwear (like winter boots or ski boots) for kids in a slightly larger size, thinking they’ll be able to get two seasons of wear out of them. But unlike coats that kids can grow into, footwear needs to fit properly right away. Properly fitted skates and boots can help prevent blisters, chafing, and ankle or foot injuries. Likewise, if socks are too small, they can force toes to bunch together, and that friction can cause painful blisters or corns.

Finally—and although this one seems like it should go without saying, it bears spelling out—don’t try to tip-toe through winter snow, ice, and temperatures in summer-appropriate footwear. “More than one news show across the country aired images of people in sneakers, sandals, and even flip-flops during the severe cold snap that hit the country in early January,” . “Exposing feet to extreme temperatures means risking frostbite and injury. Choose winter footwear that will keep your feet warm, dry, and well-supported.” Reference apma.org

Diabetes can cause atherosclerosis, or the buildup of plaque that causes the arteries to harden and narrow. This buildup results in a reduction of blood flow to the legs and feet, which is commonly referred to as poor circulation.

“Your feet are an early warning system for vascular disease,” said APMA President R. Daniel Davis, DPM. “That is why podiatrists are often the first physicians to spot vascular disease. It’s also why it is so important to include a podiatrist as part of your diabetes management team. Vascular disease is serious and can increase your risk for amputation. If your condition worsens, it may be necessary for your podiatrist to refer you to a vascular surgeon.”

Plantar Fasciitis

What is Plantar Fasciitis ?

Plantar fasciitis (PLAN-tur fas-e-I-tis) is one of the most common causes of heel pain. It involves inflammation of a thick band of tissue that runs across the bottom of your foot and connects your heel bone to your toes (plantar fascia).

Plantar fasciitis commonly causes stabbing pain that usually occurs with your first steps in the morning. As you get up and move more, the pain normally decreases, but it might return after long periods of standing or after rising from sitting.

Plantar fasciitis is more common in runners. In addition, people who are overweight and those who wear shoes with inadequate support have an increased risk of plantar fasciitis.

Plantar Fasciitis Symptoms

  Plantar fasciitis typically causes a stabbing pain in the bottom of your foot near the heel. The pain is usually the worst with the first few steps after awakening, although it can also be triggered by long periods of standing or rising from sitting. The pain is usually worse after exercise, not during it.

Plantar Fasciitis Causes

Under normal circumstances, your plantar fascia acts like a shock-absorbing bowstring, supporting the arch in your foot. If tension and stress on that bowstring become too great, small tears can arise in the fascia. Repetitive stretching and tearing can cause the fascia to become irritated or inflamed, though in many cases of plantar fasciitis, the cause isn’t clear.

Risk factors

Though plantar fasciitis can arise without an obvious cause, factors that can increase your risk of developing plantar fasciitis include:

  • Age. Plantar fasciitis is most common between the ages of 40 and 60.
  • Certain types of exercise. Activities that place a lot of stress on your heel and attached tissue — such as long-distance running, ballistic jumping activities, ballet dancing and aerobic dance — can contribute to an earlier onset of plantar fasciitis.
  • Foot mechanics. Being flat-footed, having a high arch or even having an abnormal pattern of walking can affect the way weight is distributed when you’re standing and put added stress on the plantar fascia.
  • Obesity. Excess pounds put extra stress on your plantar fascia.
  • Occupations that keep you on your feet. Factory workers, teachers and others who spend most of their work hours walking or standing on hard surfaces can damage their plantar fascia.

Black ToeWhat is a Black Toe ?

Black toe is a relatively common condition among vigorous walkers, runners, hikers, and participants in other sports and activities where the feet are subjected to stress and strain, such as football, basketball, track, baseball and soccer. The condition generally is the result of impact trauma, particularly in the big toe.

People who walk and run on hard surfaces such as concrete and asphalt are more likely to get black toe than those who exercise on natural surfaces like grass or softer man-made surfaces. Hikers can experience black toe because of the toes banging against the front area of their shoes or boots, especially on downhill sections of trails.

Symptoms of Black Toe

The most obvious symptom of black toe is a black or dark blue color on part or all the toenail caused by bruising or pooling of blood under the nail. Given that black toe most frequently is a result of trauma to the toe or the nail, it may be accompanied by pain or discomfort that usually diminishes within a few days. Black toe can also occur after lengthy periods of vigorous activity even without noticeable trauma or pain.

In some cases, discoloration of the toenail can result from causes other than black toe, some of which are benign. Others, such as the rare disorder, subungual melanoma, are cause for concern

The most common cause of black toe is repeated trauma from the toe banging against the end of the shoe. Since the big toe is usually the longest toe, the banging produces bleeding and bruising of the nail, causing it to become discolored (often blue and then black). The banging of the toe against the front of the shoe or boot is usually a sign that the shoes are too short, or that they fit too tightly and do not allow sufficient room for the feet to move dynamically during walking and running.

Other causes may include the following:

  •     Sudden trauma or injury to the toe or toenail.
  •     Fungal infection.
  •     “Morton’s toe,” a condition in which an individual’s second toe is longer than the big toe.
  •     Rarely, a subungual (occurring under the toenail) melanoma, a form of skin cancer.

foot and ankleIn our experience, with the foot and ankle, we witness children between 10 and 14 experience heel pain as a result of inflammation of the growth plate at the back of the heel. The Achilles tendon, which is the largest and strongest tendon in the body, inserts into the back of the heel bone, just behind the growth plate. Kids in track, soccer, and gymnastics will often develop this condition, known as “Sever’s disease”.

Foot and Ankle Problems in Georgia Sports

In addition, Podiatry Group of Georgia points out that runners and those frequently walking on a treadmill can both develop a stress fracture. This occurs when someone abruptly increases their mileage or number of times per week they are exercising. A stress fracture is usually an overuse type of injury that produces acute bone swelling leading to a crack in the bone. They are treated with a period of non-weight bearing to the affected area, and usually require crutches and a cast or cast boot until the bone heals.

Tendinitis can develop with an increase in mileage or with any repetitive activity. In the foot and ankle, we see tendonitis develop in the Achilles tendon, or on the inside or outside of the ankle depending on the foot type and mechanics. If you have an area of your ankle that swells up with activity, and becomes sore, you probably have a case of tendonitis. Controlling foot mechanics with the proper shoes and arch supports can often prevent or be used to treat a case of tendonitis.

How to Determine Your Foot Type for Running ShoesRunning Shoes

Podiatry Group of Georgia explains how to purchase the correct type of running shoes.  Initially, one should determine what kind of feet you have.  To determine this, you can go to a store that specializes in running shoe apparel or you can pursue it on your own.  One way is to scrutinize your footprint by either running in the sand or on paper with wet feet.

To begin, there are three types of feet.

Running Shoes for Flat Feet

Flat feet is when you look at your feet and do not notice any type if arch. From your toes to your heel on the bottom is entirely flat. In other words, an inward curve is not visible from your big toe to your heel.  Being flat-footed translates into being an overpronator in that your feet roll inward when you run. To compensate for this, this type of runner requires shoes that provide stability and some need custom made shoes (orthotics).

Running Shoes for High-arched Feet

High-arched feet is when you notice a high and definite arch on your foot. On you print test, it will curve inward causing the middle part of your foot appear skinny. Also, if you apply pressure against the bottom your foot, your arch will stay rigid. By having high-arched feet means that this person supinate or underpronate which causes your feet to roll outwards when you run. Over time the arches will collapse somewhat with runners making their feet longer. This type of runner requires flexible running shoes with a soft midsole a so as to absorb shock.

Running Shoes for Normal Feet

If you have normal feet which are neither flat-footed nor high-arched, you will notice a curve inward but not exceeding ¾ of an inch. When purchasing shoes, there is an extensive variety but stay away from running shoes that have excessive stability or motion control.
